چکیده

Abstract Introduction Patients with aortic stenosis often develop hypertrophy and fibrosis, regardless of symptoms. Cardiac magnetic resonance (CMR) represents the gold standard for evaluation fibrosis despite numerous limitations: cost, availability, atrial fibrillation, claustrophobia, kidney failure or inability to apnea. Aim The aim is validate role echocardiographic parameters, such as Global Longitudinal Strain (GLS), early marker fibrosis. Clinical laboratory data, particularly BNP, were also analyzed. Material Methods In our study we recruited 33 patients severe stenosis, comparing values GLS those Late Gadolinium Enhancement (LGE) T1 mapping CMR. Results 70% an alteration had LGE+. Univariate logistic regression shows that factors associated presence LGE on CMR are hypertension (p = 0.043), 0.032) elevated Pro-BNP 0.021); GLS, odds ratio (OR) 5 so chance finding increases times in altered GLS. multivariate analysis confirms association impaired 0.033 =) 0.025), but not values. Conclusions between can help identify earlier structural changes caused by disease, who could benefit from intervention. It remains be established, through a longer follow-up, how these alterations influence outcome suffering stenosis.

Myocardial extracellular volume fraction and myocardial fibrosis by cmr in patients with severe aortic stenosis

Background Severe aortic stenosis (AS) induces diffuse interstitial myocardial fibrosis (MF). CMR late gadolinium enhancement (LGE) technique is useful to detect focal MF. The amount of MF is associated with worse long-term prognosis after aortic valve replacement surgery (AVR). Measurements of T1 relaxation times before and after gadolinium administration allow determination of myocardial extr...

Evaluation of myocardial fibrosis with cardiac magnetic resonance contrast-enhanced t1 mapping in adults patients with aortic stenosis

Background Development of diffuse myocardial fibrosis in aortic stenosis (AS) has been associated with an adverse prognosis. Measurement of extracellular volume (ECV) from T1 mapping contrast magnetic resonance (CMR), has been recently validated as a non-invasive method for assessment of such diffuse myocardial fibrosis. Our objective was to evaluate the functional consequences of diffuse myocar...

The Incremental Effect of Obesity on Myocardial Fibrosis In Patients with Aortic Stenosis

There are many studies that show the myocardial fibrosis resulting from hypertension, aortic stenosis or hypertrophic cardiomyopathy, which are under the same condition with chronic pressure overloading. 1-4) Multiple factors are recommended for impaired cardiac function in patients with hyper-tension, such as inflammation, adaptive ventricular remodel-ing, increased mechanical stress inducing ...

Impact of myocardial fibrosis in patients with symptomatic severe aortic stenosis.

BACKGROUND In this prospective follow-up study, the effect of myocardial fibrosis on myocardial performance in symptomatic severe aortic stenosis was investigated, and the impact of fibrosis on clinical outcome after aortic valve replacement (AVR) was estimated. METHODS AND RESULTS Fifty-eight consecutive patients with isolated symptomatic severe aortic stenosis underwent extensive baseline c...
